Shellock, PhD, an internationally respected MRI researcher and educator, provides vital information pertaining to the latest requirements for MRI from the Joint Commission, with an emphasis on safety issues. As a member of the Joint Commission’s Ionizing Radiation & Magnetic Resonance Imaging Expert Panel, he is uniquely qualified to present this lecture. Course content includes procedures to manage patients with claustrophobia, anxiety, or emotional distress; guidelines to screen patients with medical implants and devices; recommendations to prevent ferromagnetic objects from entering the MRI environment; methods to handle patients requiring urgent or emergent medical care; and techniques to avoid problems related to acoustic noise. Cracking the Code: How to Leverage Lean Six Sigma to Improve Your Imaging Clinic

This session will describe step-by-step how UTSW employed Lean and Six Sigma tools and methodology to improve workflow at the Rogers MR Outpatient Clinic, and share what the results were of the improvement. Improvement metrics presented include: exam volume, median waiting time, P90 waiting time, % of on-time exam starts, and Press-Ganey Scores (related to waiting time). The effort was initially launched to address median patient waiting time prior to the MR exam, and to improve the percentage of exams started on time, thereby increasing the weekly exam volumes and revenues at the clinic, and improving patient satisfaction. The project employed a cross-functional team, including clinic managers, technologists, schedulers, and others, and a Lean expert from Philips, one of UTSW’s MR vendors. This cross-functional approach enabled a full analysis of the process from when patients arrive to when they depart after their completed scan.

The team began by following the DMAIC structure, completing a project charter, stakeholder analysis, and agreeing project scope with the team and leadership. From there, the team conducted an end to end process walkthrough, documenting each step in the process, variants or decision points, and noting opportunities for improvement. The team also employed tools including SIPOC and Spaghetti Diagrams. The team also analyzed baseline data to understand baseline process performance, and process capability. The team implemented 6S to standardize sub-wait and IV areas. The team also performed an extensive root cause analysis, identifying the top priorities for improvement from among the many observed issues. This preparation work culminated in a 4-day Kaizen event, in which the team focused on identifying solutions and planning implementation.

Key solution areas included: visual management board and daily huddle in the clinic with management and techs; adjusted staffing to provide more even coverage; appointment of lead tech to drive standard work and tech assignments; a formal process to handle late arrivals; improved signage and wayfinding; standardized workflow so all exams followed same process; and scheduling adjustments to improve patient compliance with start times. Process improvements focused primarily on the activity outside the scanning room. No additional service hours, equipment, or staff were deployed to achieve the improvements.

Results were increase in exam volume, decrease in median wait time, decrease in P90 wait time, increase in on-time exams, and increase in Press-Ganey scores.

A Critical MQSA Regulatory Update: 2D vs. 3D, EQUIP New Inspection Criteria, ACR 2016 Digital Mammography QC Manual

As part of a facility's mammography life cycle, it goes through two important phases - accreditation and certification. Advances in mammography digital screening technology (2D and now 3D) have muddied the waters with different approaches to these stages and our interaction and obligations with the involved entities. Nuances to the MQSA with digital updates will include personnel requirements to reduce the impact. Inspections then follow annually to ensure we are doing what we say we are doing. In late 2016 the FDA adopted a new phase in inspections with the goal of increasing early detection thru optimized image quality (IQ). The new inspection criteria EQUIP (Enhancing Quality Using the Inspection Program) MUST be implemented with inspection violations beginning in 2018. Additionally in mid-2017 your facility can choose to utilize the 2016 ACR's Digital QC manual. The new QC criteria allows for a simplified approach to QC testing by eliminating some tests and standardizing most others but your digital approach (2D vs 3D) will impact utilization. However, the manual offers insights and forms valuable to meet the EQUIP mandate. Working knowledge on the current status of each of these issues will aid you in reducing their impact while enhancing your digital program under the Mammography Quality Standards Act.

The number and type of medical imaging content being acquired and stored within a healthcare organization continues to expand. Managing these images for some organizations has become a challenge, especially for image producing service lines outside the Radiology department. Many organization have no real strategy to manage this growth and provide the necessary infrastructure, and support structure to store, manage and provide access to all imaging content acquired across the health care delivery system.

From One Size Fits NONE to Personalized Breast Screening

The critical causal and masking risk of dense breast tissue remained in the medical literature for more than a decade, until Connecticut became the first state to mandate the reporting of dense breast tissue to the patient through the mammography reporting results. As of today, twenty eight state legislatures have enacted legislation to protect women from missed, delayed and advanced stage breast cancer. A federal density reporting law is in Congress. Dr. Cappello will take you on a journey from breast cancer survivor to global patient advocacy, having lectured in Rome, Tokyo and Paris. She will highlight the research on mammography, screening and breast density as both a causal and masking risk. After more than a decade of normal mammograms, it was that tragic time in her life that she was offered personalized screening, including her risk of having dense breast tissue. Dr. Cappello will discuss how the standard of care for screening must be personalized screening for all women PRIOR to a cancer diagnosis to reduce late stage disease and mortality. Patient specific screening strategies tailored to the patient's family history, age, genetic profile and breast density must be the norm to reduce over diagnosis and improve outcomes. Ionizing radiation can be a hazard to patients, workers, and visitors. Fear of radiation presents additional challenges. Radiation’s benefits usually far exceed its risks. However, inappropriate radiation use can result in adverse events. Relevant education facilitates safety. This course presents brief practical overviews of general safety principles, staff radiation safety, and patient radiation management. It continues with examples of operational implementation in interventional radiology, and concludes with a sampling of administrative challenges. Because of anxiety, safety is often demanded irrespective of cost. Reducing radiation use by replacing obsolescent equipment can be effective. An appropriate radiation protective environment requires continuing attention and support. Regulatory compliance assures basic safety but does not guarantee either optimum safety or efficacy.
